CHW Welcome Home Basket Project

Transforming Public Health in Virginia Video

The Community Health Workers (CHWs) from the Community of Practice at the Center for Public Health Initiatives at NSU came together in March 2025 to make a meaningful difference in the lives of homeless older adults throughout Hampton Roads. 

Our CHWs collected and donated essential household items to the Homeless Older Adults Program at Senior Services of Southeastern Virginia through the Welcome Home Basket Project. These baskets provide seniors transitioning into housing with the everyday necessities they need to feel at home — offering comfort, dignity, and a true fresh start. 

With the number of homeless seniors in our community continuing to rise and resources remaining limited, the Welcome Home Basket Project is a powerful demonstration of what Community Health Workers do every day to address the social determinants that impact the health and well-being of our most vulnerable neighbors. 

Each basket delivered is a reminder that our community sees, values, and supports those striving toward housing stability and independence.
